What to record
Write the decision, reasons, key assumptions, probability estimate and review date. Do not write an essay; one minute is enough.
Separate process from outcome
A good decision can produce a bad outcome by chance, while a poor decision can get lucky. Review information quality and reasoning, not only the result.
Weekly review
Once a week, review one decision. Which assumption was wrong? Which signal did you miss? What will you ask earlier next time?
